Membership

Is it related to any subscription fee?
No. Your registration is only to allow us to invite you to survyes and deliver vouchers.

How much can I earn?
100 CZK per each fully completed survey and a bonus of 100 CZK at 3rd, 6th, 9th 12th survey, if you have done all the 3 consecutive waves by this point; up to 1600 CZK for the whole program. Incentives are just for completing surveys, not related to any expected purchasing or usage.

How often can I receive the incentives?
After the of 3^rd, 6^th, 9^th, 12^th survey we calculate how many questionnaires you have completed for this 3 surveys period - 1,2, or 3- and email ALZA vouchers earned.

What if I miss a survey?
You will not receive a voucher for it and will miss the chance for the 3 surveys bonus for that period.

What type of incentive?
ALZA Electronic Vouchers of 100 CZK per survey for 3 completed + bonus after the of 3^rd, 6^th, 9^th, 12^th survey - 4 (four) ALZA Electronic Vouchers of 100 CZK.

How do I know when I will get incentive?
We will email you the voucher codes after the 3 surveys period. You can also check status in your account here at cz.mytobaccostudy.com.

What if I unsubscribe from My Tobacco Study?
We will calculate and send any due incentives in the same way - after the end of 3^rd, 6^th, 9^th, 12^th survey, respectively.
